About The HART Hub

The HART Hub is co-led by Hôtel-Dieu Grace Healthcare (HDGH), House of Sophrosyne (HoS), and the Windsor Essex Community Health Centre (weCHC).

Bringing together multiple community partners, the HART Hub delivers person-centred, integrated care that helps people access the health, housing, and social supports they need — promoting recovery, stability, and long-term well-being.

What Is Available Now

HART Hub Windsor-Essex provides encompassing supports for people 16 years and older who are experiencing homelessness and want help with substance use, mental health, and housing.

Comprehensive medical supports to ensure clients have their healthcare needs met.

  • Specialist referral
  • Referral to supportive community programs
Outreach and Social Support

Outreach and Social Support

Assisting clients with navigating systems and accessing the resources that they need.

  • Community engagement and outreach encouraging change

  • Judicial navigation and support
  • ID and birth certificate support
  • Liaison between social services and housing
  • Subsidy and housing application
  • Ongoing caseworker support and engagement
Mental Health Navigation

Mental Health Navigation & Day Treatment

Providing mental health and addictions care to clients, including bed-based treatment.

  • 24/7 bedded live-in treatment
  • Trauma-based treatment and peer-led programming
  • Recreational and leisure activities programming
  • Mental Health and Addictions referral
  • 1-on-1 counselling
  • Ongoing mental health case management
